Accident management

1. Add a new Safety accident or Fire event

Add a new Safety accident or Fire event

Click New+ button above the table to add a new Safety accident or Fire event. In the new window complete all required data by this way you are creating you`re A1 report, when you complete all data, click Save when you want just to save it and keep working on this event later or, you will be still able to modify data and the event is visible for you only. When you click Submit, you will not be able to modify data anymore and the event will be visible for the Global manager waiting for approval / rejection.

Now you check the status of your A1 report in the A1 / Fire report deadline / status, when it is submitted you will see the yellow status, place your mouse on the color dot to check the status description.

A2 / A3 report will is not available to complete data until A1 report approval.

When A1 report has green status, Approved, you can complete data of A2&A3 report, click on magnyfiying glass icon in the Detail column.

New window appears and you have to choose Cause nature, insert description of the current and ideal status, then add a problem for which you will make a 5Why analysis. Click Save when all data inserted.

Now you have to add countermeasure/s and make 5Why analysis for the problem you`ve just inserted. It is mandatory to make the 5Why analysis until the 3rd Why. When data is completed, you can save it and implement countermeasures, in this case, your A2&A3 report will be visible for you only and you will be able to modify data. When you click Submit, data modification will not be possible anymore and the report will become available to the Global manager waiting for apporval / rejection.

Now you have to add countermeasure/s and make 5Why analysis for the problem you`ve just inserted. It is mandatory to make the 5Why analysis until the 3rd Why. When data is completed, you can save it and implement countermeasures, in this case, your A2&A3 report will be visible for you only and you will be able to modify data. When you click Submit, data modification will not be possible anymore and the report will become available to the Global manager waiting for apporval / rejection.

When A2&A3 report has red status is means that it is rejected, place your mouse on the red dot to see the comment, change data and submit it once again. When the status is green, it means that is it approved and no other action is required.

When A1 and A2&A3 reports are approved, you can view A4 report, go into the detail of a given accident and select the last tab A4 report. You can only view data here, no modification is allowed.

Click on the Accident management widget to access the main page.

In the main page, click on the "TEC report" button to download the summary export. The Summary export allows extraction of all the accidents from the page in a single report.

4. Overview, filter and order the data

Overview the data of all accidents with the focus on: description, part of the body and effects. Moreover filter and order the data presented in the page.

In the main page all the accudents can be seen with the information such as: FY, date, tape, event type, etc.

The detailed data can be seen in the columns: description, part of the body and effects. For example, point your mouse on the human figure in the column "Part of the body" and a tooltip with the detailed information will appear.

To filter the data, select a desired filters and click on the button "Apply". The information in the main table will be displayed based on the desired filters.

5. Define and update problems of an event

Problems of an event can be defined and modified within the tab Problems. They can be further divided into problem groups and differenciated by the cause nature.

Select the desired accident and click on the detail (lens icon) to proceed to the detail page.

6. Manage countermeasures

Countermeasures can be managed after the 5 Whys analysis is completed.

Click on the widget Accident management to proceed to the main page.

In the main page select a desired accident and click on the lens icon to proceed to the detail page.

In the detail page click on the tab Causes.

To manage countermeasures select a problem with the completed 5 Whys analysis (the problem with the green indicator) and click on the "+" sign in the column "Countermeasures".

In the pop-up Manage Countermeasures, define: desctiption, responsable person and the date. After that click on the button "Save".

By pointing the mous on the "+" sign in the column "Countermeasures" a tooltip will appear indicating how many countermeasures are managed.

7. Download documentation of the accident / fire

Download documentation of the accident / fire

Anytime you need to, you can download all reports relative to a given accident in the Excel file, go into the detail of a given accident and click Excel icon found in the right upper corner and click name of the report you want to download.

8. Check Safety accidents and Fire events status

Check Safety accidents and Fire events status

On the main page table you can quickly check status of Safety accidents and Fire events, yellow status means report submitted waiting for approval / rejection, red status means report rejected waiting for submission after corrections done, green means report approved, grey one means a report not activated yet. You can view and download summary cards, set filter, check by whom reports have been approved / rejected.

9. Reports approval / rejection

Reports approval / rejection

On the main page, check if there is any accident having yellow status (submitted) in the A1 report column. Yellow color means status submitted so waiting to be controlled for approval / rejection. Go into the detail page of a given accident.

Check all data inserted in A1 report and insert Class and grade in the bottom of the page. This data will be automatically fulfilled in the A4 report. If necessary, you can also select Serious near miss or Reference accident. Then you can choose to Save only, in this case you will be still able to modify data and control result will not be visibile for the Plant that inserted an accident. You can Approve and in this case no other action will be required, Plant will be able to complete A2&A3 report or you can Reject A1 report, you will have to add a mandatory comment which will be visible for the Plant.

On the main page table check if there is any A2&A2 report with yellow status. This status means that report is submitted so must be checked for approval /rejection. Go into detail of a given accident.

Now choose the second tab A2&A3 report.

Check all data provided and in the bottom of the page you can choose to Approve, in this case there is no other action required from the Plant or you can Reject, in this case you will have to add a mandatory comment which will be visible for the Plant.

When both reports A1 and A2&A3 have green status so both are approved, you can go into detail of a given accident and choose the last tab A4 report. Class and grade data is completed automatically when it was inserted in A1 report.

Complete the table starting to mark data from the left column.

In the bottom of the page you can just Save data, the report will be still editable and it will not be visibile for the Plant, when you Approve, the report will not be editable any more and will become available for the Plant that inserted the accident.
